PDF software with integrated merge from DBF/ODBC

I regularly need to complete PDF forms for my clients with information that I already have in my case management system. The problem is doing merges. I could create a layout in the CMS to populate a form, but I would have to do that per form. My preference is to do a database/mail merge from the CMS, either directly to a DBF table or via ODBC. PDF XChange, which I use now, has a rudimentary merge function, but the merge cannot be "saved" to the template. It has to be recreated each time the merge is done (which saves no time). There's a plug-in for Acrobat Pro that could work, but it would require two subscriptions (which I find grating). Does anyone know of a PDF editor with an integrated database/mail merge function where the merge logic can be saved to the template? Thanks!
